The free NHS Discharge Medicines Service helps to provide support and help you to manage your medicines after you have been discharged from hospital.
If there has been a change to your medicines in hospital, either a change to a pre-existing medicine or one has been added/removed, we can help you to check that the changes have been made by your GP as well as answer any questions you may have about your medicines.
To be eligible for this service, you need to have been discharged from hospital in the last four weeks and be living in England. However, if you are not eligible for this service and need help or support with your medicines, please contact our pharmacy team who will be happy to assist.
There are 3 steps to the Discharge Medicines Service:
1. Referral from the Hospital to Our Pharmacy
Your discharging hospital will send information related to your discharge, particularly related to changes, omissions and additions of medication to our pharmacy.
2. Our Pharmacist will Check Your First Prescription After Discharge
Once we receive the first prescription from your GP surgery after your discharge, we will check that the changes have been made. We will also check that you understand the changes and answer any questions that you may have.
3.Follow Up Consultation
Our pharmacist will contact you via phone or video call to check that everything is going okay in terms of your medications and answering any questions you may have about your medicines.
